Once enrolled in Medicaid and Chip, you’ll have the option of choosing from one of two dental plans: DentaQuest and MCNA. You will then need to select the main dentist. To set Perfect 32 Family Dentistry as your main dentist, you simply need to provide our number (469) 863-7009 and address 3630 N. Shiloh Road, Ste #209, Garland, TX 75044.
If your child is currently enrolled, but you wish to change to Perfect 32 Family Dentistry as your main dentist, you can do that at our office when you come in for your kid’s appointment or you can call one of the following numbers:
